If your immigration process requires a medical exam, you'll hear the term civil surgeon. Choosing this doctor well matters more than it seems: an exam done by the wrong physician simply doesn't count. Here's what a civil surgeon is, how to verify one, and what to expect from the visit.
What is a civil surgeon?
A civil surgeon is a physician authorized by USCIS — U.S. Citizenship and Immigration Services — to perform the immigration medical exam (Form I-693) inside the United States. The name is confusing: they don't "operate" on anyone; it's simply the official title USCIS gives to physicians designated for this process.
Not every doctor qualifies. Your regular physician, however good, cannot sign a valid I-693 without the USCIS designation.
Why does choosing well matter so much?
Because USCIS only accepts the form completed, signed and sealed by a civil surgeon. If a non-authorized doctor performs it:
- The form will be rejected, even if the exam itself was done correctly
- You pay twice: once for the invalid exam and again for the repeat
- Your case can be delayed weeks or months over an avoidable RFE (request for evidence)
In a process where every document counts, this is a completely preventable mistake.
What the civil surgeon does during the exam
- Reviews your medical and vaccine history, including records you bring from your home country
- Performs the general physical exam
- Orders the tests USCIS requires, such as tuberculosis screening and the blood and urine tests that apply to your age
- Administers any vaccines you're missing, at the same clinic when possible
- Completes and signs Form I-693 and hands it to you in a sealed envelope you must not open

How to verify a doctor is authorized
- Use the official USCIS locator: uscis.gov offers the "Find a Doctor" tool, where you can search civil surgeons by ZIP code
- Ask the clinic directly whether a USCIS-authorized physician performs the exam and whether they deliver the sealed envelope
- Be wary of anyone who can't confirm it clearly or offers to "fix" the form without an exam

- Do you serve patients in Spanish? The exam includes questions about your health history; understanding them well prevents errors on the form.
- Do you give vaccines the same day? If you're missing one, solving it in the same visit saves you trips.
- How much does it cost and what does the price include? A serious clinic tells you upfront, lab tests included.

Frequently asked questions
Are a civil surgeon and a panel physician the same thing? No. The civil surgeon performs the exam inside the United States (for example, for adjustment of status). The panel physician performs it abroad, for consular processing. If your process is inside the U.S., you need a civil surgeon.
Does passing the medical exam guarantee my case is approved? No; it's just one requirement. The civil surgeon certifies your health status, and USCIS evaluates the whole case. For legal questions, talk to your immigration attorney.
Can I open the sealed envelope to check that everything is right? Don't open it: an opened envelope can invalidate the form. Ask the clinic for a copy of your I-693 for your records — it's your right, and we always provide one.
