Put the boxes of every medicine you take in a bag, vitamins and herbal remedies included, and if you own a glucose meter or home blood pressure cuff, bring your logged readings on paper or on your phone.

What happens once your results come back?

1. We let you know they're in and go over each value with you, side by side with your previous check.
2. The clinic's medical team decides whether your treatment stays the same, gets adjusted or changes.
3. You leave with concrete goals: what to swap on your plate, how much to move and when to measure again.
4. If you need medication, we send the prescription electronically to the pharmacy you prefer.

Why isn't feeling fine enough?
