Immigration Medical Exam I-693

Immigration medical exam with a USCIS-authorized physician and the sealed Form I-693.

Immigration Medical Exam I-693
  • Authorized civil surgeon
  • Sealed Form I-693
  • Required vaccines available
  • Process explained in Spanish

The I-693 immigration medical exam is the health step USCIS requires from anyone applying for adjustment of status. At Clínica Hispana Nueva Salud Hammerly in Spring Branch, Houston, a USCIS-authorized Civil Surgeon performs it, and you leave with the form sealed inside a closed envelope once everything is complete.

What happens during the I-693 visit?

  1. You walk in to 8538 Hammerly Blvd Suite B and check in at the front desk.
  2. The Civil Surgeon goes over your health history and whatever vaccine record you bring.
  3. You get the physical review the form calls for.
  4. Lab samples USCIS requires for your age are collected, including the tuberculosis screening.
  5. If a required vaccine is missing, you're told which one; flu and tetanus shots are given right here through our vaccine service.
  6. When the results are in, we let you know so you can pick up the envelope.

Which papers should you bring?

  • A passport or another official photo ID.
  • Your vaccine card or record from any country, even if it has gaps.
  • A list of the medicines you take and any past diagnoses.
  • Proof of treatment if you were ever treated for tuberculosis.
  • The I-693 printed from the USCIS website, unsigned, because you sign it in front of the Civil Surgeon.

What do you do with the sealed envelope?

Keep it closed. USCIS rejects an I-693 whose envelope arrives opened or tampered with, so you store it as it is and submit it with your application or when USCIS asks for it. You also get a copy for your personal records.

What if a result comes back abnormal?

An out-of-range result doesn't automatically stall your case. The clinic's medical team walks you through what was found, what the next step is and, when needed, how the form gets finished after the extra evaluation.

To start your immigration medical exam, stop by Hammerly Blvd any day of the week or call and ask about the I-693 price before you come.
